Care Manager for Evergreen Site

Full time with benefits -Care Manager/CHORE Supervisor. Minimum three years comprehensive care management experience with a preference of working with the elderly.  Non-profit org. seeks individual with prior experience working with consumers to understand and impact key issues related to successful aging.  Assist consumers in assessing needs, developing & implementing a  care plan to access necessary  resources that aid the client in remaining at home in their own communities.  Requires extensive  knowledge of home and community based services for older adults. Must enjoy working collaboratively. Extensive working knowledge of issues related to aging; community resources, health care professionals and systems.  Solid interpersonal, organizational and writing skills.  Strong initiative & judgement abilities; compassionate, analytical, & comprehensive approach to working effectively with consumer & care givers including: health professionals, community resource providers, & families.  Must be a creative & effective problem solver who strives for excellence and seeks positive & proactive solutions.   General office skills also required including computer, writing reports, gathering and documenting data. Must be able to work independently. Background and motor vehicle check will be conducted. Must have own means of transportation for work related travel within the mountain communities. CHORES Direct Service Provider

Part time - Seniors’ Resource Center CHORE Services.  Staff needed to serve the mountain community in and around Evergreen in Clear Creek and Gilpin Counties. Applicants must have reliable transportation.

Seeking hard working, committed, team players with initiative, a sense of humor, and strong interpersonal abilities with seniors. Minimum Qualifications: High School Diploma. Responsibilities include the following: Performs a variety of heavy household cleaning duties for clients including deep cleaning of bathroom, kitchen floors, stove and refrigerator, vacuuming rugs, clean up and removal of trash, organizing and packing for home inspections or moves, unpacking following moves, yard work such as raking and general clean up, snow shoveling of primary exits, and cleaning of closets and stage areas. Required Knowledge, Skills, and Abilities include: familiarity with issues and problems related to senior and disabled citizens; knowledge of cleaning procedures, and skilled in the use of housekeeping materials and general outdoor work tools. Drivers Needed

Seniors’ Resource Center is seeking multiple part time drivers for its non-profit transportation system serving the elderly and disabled. Knowledge of  the rural areas including Jefferson, Park, Gilpin and Clear Creek counties.  Clear Background and good Motor Vehicle Record required. CDL not required, but a strong plus. Previous driving experience beneficial. Positions are based out of our Wheat Ridge office. Part time, flexible hours - Seeking committed, very dependable, friendly, compassionate individuals to assist seniors in their homes.  No experience necessary, training provided. Seniors' Resource Center is seeking caring, mature, committed, friendly, flexible, compassionate team member for our Adult Day & Respite Program in Wheat Ridge, CO.  We need enthusiastic PRN CNAs who have experience in leading groups and activities along with providing personal care to older adults with memory loss. Candidates must be available during most business hours (Monday-Friday 7:30am-5:30pm). Day Enrichment Program Support (Volunteer)

Make a difference in the lives of the aging and disabled in your community. Join the Adult Day and Respite Services (ADRS) team at the Seniors’ Resource Center (SRC) as a Day Enrichment Program Support Volunteer. The Day Enrichment Program Support Volunteer will provide meaningful one to one interaction, support participants in creative group projects as well as assist staff with planned and spontaneous activities thus enhancing the participant’s daily experience.
