Reaching Out, Meeting Needs

Are you concerned about a senior? The following signs could indicate that an older adult is in need of help.

Mental: Confusion, disorientation, memory loss, repetitiveness
Physical: Difficulty seeing, hearing or walking, alcohol or substance abuse
Emotional: Recent loss of spouse, appears depressed, anxious or withdrawn, loss of appetite
Social: Isolated, possible victim of abuse, neglect or exploitation
Environment: Home needs repair, trash or debris in yard, neglected pets
Economic: Confused about money matters and unable to afford necessities

Contact our Senior Reach call center to discuss your concerns and make a referral. It’s a simple way to help older adults get assistance before a crisis develops.

Senior Reach Services include:

  • Counseling
  • Care management
  • Wellness coaching
  • Depression screening and treatment
  • Identification and early intervention of prescription drug misuse or abuse

Serving Adults 60 and older in Boulder, Broomfield, Jefferson, Clear Creek and Gilpin counties.

Senior Reach is an award winning, innovative collaboration between Seniors’ Resource Center, Jefferson Center for Mental Health, and Mental Health Partners, and  has proven highly successful in decreasing depression, anxiety, feelings of hopelessness, and social isolation among seniors.

Our mission is to support the well-being, independence and dignity of older adults by educating the community, providing care management and mental health services, and connecting older adults to community resources.
