Heather Logan is the Director of Accountable Care at Metro Community Provider Network (MCPN), where she focuses on hospital utilization project management, comprehensive care coordination development, program operations, and external relations. Heather is also the Director for the Bridges to Care Program; which is an innovative hotspotting program that focuses on lowering inappropriate use of the emergency room and increased access into primary care .The program currently partners with 3 hospital systems and has a cost savings of $8 million dollars. Heather holds a Master’s in Social Work from Colorado State University. She currently serves as the Chair of the Jefferson County Hotspotting Alliance and is on the board of Aurora Health Access and University Hospital’s Community Health Needs advisory council. Heather is passionate about integrated, all-inclusive health care and advocating for individualized care and quality of life. Heather is passionate about older adults having access to the essential services they need in order to live a happy and healthy life at home. Seniors’ Resource Center tailors all of its services to meet the individual needs of each client served. SRC is not only dedicated to treating each client with dignity and respect, but going above and beyond to make each client feel special and cared for. Heather enjoys yoga, crocheting, running and chasing after her 2 year old daughter, Keva with husband Matt!
